Applications available for FWISD free, reduced-price meal programs


Information and applications for the Fort Worth Independent School District's meals provided under the National School Lunch, School Breakfast and After School Snack programs will be sent home with all FWISD students during the first week of school.

Additional applications will be available in each school's office and cafeteria and in the lunch card office, 601 E. North Side Drive, Fort Worth, Texas 76106. Beginning July 5, family members may also apply in person at the lunch card office between 8 a.m. and 4 p.m., if they so choose. Applications may be turned in to the school's cafeteria manager or mailed to the lunch card office.

Any student who would like a free or reduced-price lunch card must apply, even if the student had a card last school year.

Children must be registered in school before an application for free or reduced-price meals will be accepted. Parents will be notified within 10 days after they apply whether their children qualify for one of the national programs.

To apply, families receiving food stamps or Temporary Assistance to Needy Families (TANF) must list the child's name and food stamp or TANF case number. An adult household member must sign the application.

Applications for children not receiving food stamps or TANF must list names of all household members, their source of income and the amount, and the social security number of the adult member of the household who signs the application. If the adult household member who signs the application does not have a social security number, he or she should write the word "none" on the application.

A copy of the FWISD's free and reduced-price lunch policy is available for review in the district's nutrition services office.
