FWISD NOW OFFERING TWO LOCATIONS FOR EMPLOYEE FLU SHOTS
Fort Worth, Texas (January 15, 2013) – The Fort Worth ISD now will offer two flu shot clinics for employees and their families on Wednesday, January 23, 2013.
Passport Health – North Texas Flu Shots will be conducting vaccination clinics from 3 p.m. to 6 p.m. at both the Central Administration Building (100 N. University) and Wilkerson-Greines Activity Center (5201 C.A. Roberson).
If you missed receiving the flu shot or other vaccinations, it is not too late. All the same vaccinations available during earlier clinics will also be available this time. So, if you received a first Hepatitis B vaccination at your campus during the fall flu clinic, it is time for your second dose. You will need a third and final dose five months or more from now. You can ask your personal doctor if they can provide this or you can wait until next fall and receive it with your flu shot.

Please print the form and bring it with you to the clinic. All vaccines are on the same form this year, except B12 and Zostavax which require additional forms.

Your must bring photo copies of your TRA Active Care insurance card and driver’s license to the shot clinic to receive your vaccinations at no charge.
If you need a record of previous vaccines received from North Texas Flu Shots, please send an email to info@northtexasflushots.com. Include your name, TRS ActiveCare Insurance ID number and the email address to which you want the records sent. Please do this at least five days prior to the clinic to allow time for response.
